Ramazzotti traces back to 1815, when Ausano Ramazzotti, a young herbalist working in a small Milan laboratory, began experimenting with sweet orange peels, gentian, rhubarb, turmeric, and rosemary sourced from around the world, at a time when political instability made such ingredients hard to come by in the city. His blend of thirty three botanicals caught on quickly, and in 1848 he opened a bar near Milan's La Scala opera house where patrons could drink his amaro alongside their coffee, effectively launching Italy's first non-wine-based liqueur. The brand has been part of the Pernod Ricard group since 1985, with production and bottling now carried out at a facility in Canelli, in Piedmont.

The recipe remains a closely guarded secret passed down since Ausano's time, built around thirty three herbs, roots, flowers, and fruits, with orange peel processed separately through distillation while the rest of the botanicals are macerated and infused into the base spirit. No artificial coloring or additives are used, and the exact proportions are known to only a handful of people within the company.
